Cooled radiofrequency ablation switches off the small nerves that carry pain signals from your knee. The joint itself is not touched. In a randomised trial, 74% of patients had at least half their pain gone at six months, against 16% of those given a steroid injection, and about 65% still had that relief a year later after a single treatment. It costs ₹80,000 to ₹2.2 lakh in India.

There is a large group of people who fall between the available answers. Their arthritis is too advanced for physiotherapy and injections to do much. They are not ready for a replacement, or a heart condition makes a major operation risky, or they simply do not want one. They go from doctor to doctor being told there is nothing much to offer.
This is the treatment for that group, and the reason you have probably not heard of it is that it is done by relatively few doctors and marketed by almost none.
Told surgery is your only option, or not medically fit for a knee replacement? Learn if Cooled RFA can switch off your knee pain — free.
What It Actually Does: Deactivating the Genicular Nerves
Four or five small sensory nerves, called the genicular nerves, carry pain signals from your knee capsule up to your brain. They do nothing else. They do not control muscle movement, they do not provide joint stability, and you do not need them for your knee to walk or function.
In this procedure, fine needles are positioned close to these sensory nerves under precision X-ray guidance (fluoroscopy), and a controlled amount of radiofrequency energy deactivates them. The pain signal stops arriving at the brain. The knee joint capsule is never entered.
📌 The essential distinction: Cooled RFA does not cure your arthritis or regrow cartilage. The joint continues to wear underneath. What changes is that you stop feeling the chronic pain. For someone whose primary daily problem is unremitting knee pain that disrupts sleep and walking, that distinction is transformative.
What the Clinical Evidence Shows: 74% Relief at 6 Months
Cooled RFA is one of the most rigorously evidenced minimally invasive procedures in joint care, which is remarkable for a treatment so few patients have heard of.
In a prospective, multicentre randomised clinical trial published in Regional Anesthesia and Pain Medicine, 151 patients with chronic knee osteoarthritis were randomised to receive either Cooled Radiofrequency Ablation or standard intra-articular steroid injections:
- At 6 months: 74% of the Cooled RFA group achieved ≥50% pain reduction, compared to just 16% in the steroid injection group.
- At 12 months: Approximately 65% of patients maintained significant pain relief after a single session.
- At 18 to 24 months: In extended follow-up cohorts, 69% sustained relief at 18 months and 63% at 24 months, with marked improvements in physical function and Oxford Knee Scores, and zero serious device-related complications.
The Two Honest Limits You Must Know Before Deciding
Every medical procedure has limits, and patients must know them beforehand:
- It buys time; it does not stop the clock: Your arthritis progresses underneath. If your knee will eventually require joint replacement, Cooled RFA delays that conversation for 1 to 3 years rather than eliminating it. For a 60-year-old wanting to reach retirement or someone waiting for a cardiac condition to stabilize, that delay is the entire goal. For someone whose joint is mechanically collapsing, it is not.
- Nerves naturally regenerate: Sensory nerves slowly regrow over 12 to 24 months. That is why relief gradually fades rather than lasting indefinitely. When pain returns, the procedure can be repeated safely.
⚠️ Practical post-op expectation: Expect approximately 10 to 14 days of local procedural soreness and aching around the needle entry sites before the nerve deactivation takes full effect. If nobody prepares you for this, the first fortnight can be alarming. Once that initial soreness settles, the pain relief becomes clear.
Who It Suits — And Who It Does Not
| Likely Suitable Candidate | Not Suitable (Better Alternatives) |
|---|---|
| Grade 3 or Grade 4 arthritis where physiotherapy and intra-articular injections have stopped helping | Grade 1 or Grade 2 arthritis where conservative therapy, muscle strengthening, or simple steroid injections remain effective |
| High surgical risk patients with heart disease, prior bypass surgery, renal impairment, lung disease, or severe obesity | Mechanically unstable knees that are buckling, locking, or displaying severe angular collapse (>15° deformity) |
| Patients seeking to defer replacement for career, family, or personal reasons for 1 to 3 years | Active candidates ready for definitive surgery who have no health contraindications to a knee replacement |
| Persistent pain after knee replacement (post-surgical neuropathic joint pain with intact implants) | Active local skin infection or systemic infection at the time of procedure |
Why "Cooled" Is Not Just a Marketing Word
Older conventional radiofrequency ablation (RFA) heated tissue directly through a bare metal needle. The heat spread in a narrow, line-shaped burn. This had two major drawbacks: if the needle was positioned just 2 millimetres off the nerve, it missed entirely; and the extreme focal heat risked charring tissue and causing skin burns.
Cooled RFA circulates sterile water continuously through the probe while delivering RF energy. This water cooling prevents tissue charring at the probe tip and creates a spherical treatment zone up to 10mm in volume. Because the treatment field is a sphere rather than a narrow line, it reliably captures the nerve even with slight anatomical variations, providing superior clinical accuracy and virtually eliminating skin burns.
What Happens on the Day of Treatment
Cooled RFA is performed as an outpatient day-care procedure in an operating theatre or catheterization lab:
- Image Guidance: The interventional doctor uses real-time fluoroscopic X-rays to pinpoint the bony landmarks where the genicular nerves run.
- Local Anaesthesia: Local numbing medicine is applied. You remain awake and comfortable throughout.
- Radiofrequency Cycle: Fine needles are introduced and RF energy is applied for approximately 2 to 3 minutes per nerve branch.
- Discharge: The entire procedure takes under 45 minutes. After 1 to 2 hours of post-procedure observation, you walk out and return home the same day. There is no surgical cut, no stitches, and no overnight hospital stay.
What Does Cooled RFA Cost in Mumbai & Pune?
| Setting | Indicative Package Cost (One Knee) | Key Features |
|---|---|---|
| Quality-Screened Mid-Tier Hospital / Day-Care Centre | ₹80,000–₹1,10,000 | Full single-use cooled probe kit, fluoroscopy guidance, day-care admission |
| Large Corporate Hospital (TPA / Insurance Route) | ₹1,60,000–₹2,20,000 | Same consumables, higher room tariff, corporate facility overheads |
The cost is primarily driven by the single-use proprietary cooled RF probes and fluid circuitry, which cost ₹30,000 to ₹40,000 per patient before facility fees. This is why having Cooled RFA at a screened mid-tier day-care centre provides the exact same clinical quality and identical single-use technology as a large corporate hospital, at roughly half the total bill.

Will Health Insurance Cover It in India?
Most comprehensive health insurance policies in India cover Cooled Radiofrequency Ablation when performed as a necessary day-care procedure in an empanelled hospital under medical indication. However, because some claims desks are less familiar with genicular ablation than conventional joint replacement, formal cashless pre-authorization with detailed clinical notes is essential.
At MyDocsy, our dedicated care navigators coordinate directly with your insurer's TPA desk to secure pre-approval before your procedure date.
Three Questions to Ask Before Choosing Cooled RFA
- "Are you using cooled radiofrequency technology or older thermal RF?" (Cooled RFA creates a substantially larger lesion and higher reliability).
- "Given my X-ray grade and knee alignment, how much pain reduction can I realistically expect?"
- "If the relief fades in 18 months, can the procedure be safely repeated?" (The answer should be yes).
Evidence and Clinical References
- Prospective Multicentre Randomised Controlled Trial (151 patients, Regional Anesthesia & Pain Medicine): Demonstrated that 74.1% of patients receiving Cooled RFA achieved ≥50% pain relief at 6 months compared to only 16.2% receiving intra-articular steroid injections.
- 12-to-24 Month Clinical Extension Study: Showed sustained ≥50% pain relief in 65% of patients at 12 months, 69% at 18 months, and 63% at 24 months, with significant improvements in Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) functional scores.
